The Injury Calendar: When Things Go Wrong in a 5K Block

Running injuries aren’t randomly distributed across a training block. They cluster.

Different tissues fail at different points, for reasons that are fairly predictable once you know the pattern — and knowing which week you’re in tells you what to watch for before it becomes a problem.

Here’s the map of an eight-week 5K build, week by week, with what typically shows up and what it means.

But first, one finding that changes how you should read the whole thing.

The Number That Actually Predicts Injury

For decades runners have been told to follow the 10% rule — don’t increase weekly mileage more than 10% over the previous week. It’s in training plans and built into watch algorithms.

An 18-month prospective study of 5,205 runners across 87 countries, tracking 588,071 running sessions via GPS watches, tested three different metrics against actual injuries: week-to-week mileage change, the acute-to-chronic workload ratio, and single-session distance relative to the runner’s longest run in the previous 30 days.

Only the third one predicted injury.

When runners exceeded roughly 110% of their longest run in the past 30 days, risk rose sharply — a modest spike raised injury risk by about 52%, and a 30% spike raised it by around 64%. Meanwhile week-to-week mileage change and the acute-to-chronic ratio showed little predictive value.

The practical translation: your weekly total matters less than any individual run that’s substantially longer than anything you’ve done recently. A tidy weekly mileage figure can hide one dangerous session — and that single session is what your tissues actually respond to.

Keep that in mind as you read the calendar below, because most of the danger points are single sessions, not weeks.

Weeks 1–2: The Calf and Shin Window

What typically shows up: Calf tightness and soreness. Diffuse aching along the inner shin. Sore Achilles in the morning.

Why now: This is the point where cardiovascular fitness and tissue tolerance are furthest apart, particularly if you’ve had a break. Your lungs let you run further than your calves are conditioned for, and the calf-Achilles complex absorbs and generates enormous force with every stride.

What’s normal: general muscular soreness that settles within a day or two and improves week over week.

What isn’t: morning Achilles stiffness that’s worsening, or shin pain becoming more localized.

What to do: start heavy calf work now, both straight-knee and bent-knee, twice weekly. Starting it in week five is too late.

Weeks 2–3: The Foot

What typically shows up: Plantar heel pain, particularly first steps in the morning. Arch soreness. Forefoot discomfort.

Why now: Same mechanism, slightly delayed. The plantar fascia and foot structures are absorbing more load than they’ve been asked for recently.

A local factor: if you’ve spent months in flip-flops or unsupportive sandals — which in South Florida is most of the year — your feet have been operating with minimal support and are being asked for a considerable step up.

What to do: don’t stretch it aggressively and hope. Address load, footwear, and calf capacity now, while it’s a niggle.

Weeks 3–5: The Tendons Report Back

What typically shows up: Achilles pain that warms up during a run and worsens afterward. Patellar tendon pain below the kneecap. Outside-of-knee pain appearing at a predictable point in each run.

Why now: Tendons adapt considerably more slowly than muscle or cardiovascular fitness. Three to five weeks is roughly when accumulated loading starts producing symptoms in tissue that hasn’t kept up.

The critical detail: tendon problems characteristically feel better as you warm up and worse the next morning. That pattern is routinely misread as improvement, and it’s why people run on them for weeks.

Use the next-morning test. Back to baseline means the load was appropriate. Clearly worse means it wasn’t.

Weeks 4–6: The Bone Stress Window

This is the most important section on the page.

What typically shows up: Sharp, localized pain on a bone — commonly the shin, the foot, sometimes the hip or pelvis — that you can cover with a fingertip.

Why now: Bone remodels in response to loading, and there’s a phase during that process where it’s temporarily more vulnerable. Four to six weeks into a meaningful increase in loading is a classic window.

How to tell it apart from everything else: the pain worsens as the run continues rather than easing after warm-up. That’s the reverse of the tendon pattern, and it’s the single most useful distinguishing feature.

What to do: stop running and get assessed. Bone stress injuries have far better outcomes when caught early and can become considerably more serious when run through. This is not a wait-and-see situation.

Higher risk: anyone with a previous stress fracture, irregular or absent menstrual periods, restricted eating or low energy availability, or a rapid increase in training.

Weeks 5–7: Speed Work Consequences

What typically shows up: Hamstring strains and calf strains. Aggravation of anything already grumbling.

Why now: This is when most 5K plans introduce intervals and tempo work — and speed loads tissue differently from easy running. Faster running means greater force, higher rates of loading, and longer strides.

The common error: adding intensity while volume is still climbing. Progress one variable at a time.

And the spike finding applies here too: a hard session considerably longer or harder than anything you’ve done in the past month is exactly the kind of single-session jump the research identified.

Week 8: The Taper Trap

What typically shows up: Something new and unexpected in the final week, or an old niggle flaring.

Why now: Two reasons. Accumulated fatigue means everything from the previous seven weeks is present at once. And people frequently do something out of character in taper week — an extra hard session because they feel fresh, or a much longer run because there’s more time.

What to do: reduce volume by roughly 30 to 50 percent while keeping some intensity. Don’t add anything new. The fitness is already banked.

Race Day and the Week After

What typically shows up: Post-race soreness that doesn’t settle, or an injury appearing days afterward.

Why: A 5K raced properly is a maximal effort — considerably harder than anything in training. And South Florida race season runs through the cooler months, which means many people race harder than their training prepared them for.

What to do: two to three easy days minimum. Resist racing again the following weekend unless you’ve built up to it.

The Two Things That Cut Across Every Week

Strength training. A systematic review and meta-analysis of 26 randomized controlled trials found strength training reduced overuse running injuries by around 50% and acute injuries by around 30%. Notably, the same body of evidence found no significant protective benefit from stretching programs.

If you do one thing off the run, do this one.

Your injury history. Previous injury is consistently identified as the single strongest predictor of future injury. If you had plantar heel pain last season, that tissue remains more vulnerable — and it deserves specific attention in week one rather than week five.

Symptoms That Mean Stop, Not Adjust

Sharp, pinpoint pain on a bone that worsens as you run. Pain that changes how you move. A pop or sudden sharp pain. Pain at rest or at night. Swelling. Numbness or tingling. Any pain persisting beyond two weeks despite backing off.
